Circulator Declarations

The shorthand is the

1. first letter of the county
2. first letter of first name
3. first letter of last name
4. first digit of house number
5. first letter of street name
6. first letter of city
7. first letter of state
8. full 5-digit zip code

If anything is crossed out and corrected but not initialed, do not enter the letter or digit, but enter a dash as if it's missing.

Enter the first and second date in the paragraph, and check the box if the circulator signed. If the signature or date is crossed out and not corrected with the circulator's initials, do not check the box or enter the date.

Check the boxes if signed and stamped or embossed by the notary, and verify the expiry  written and is a future date. Embossments are very faint. The client may write "E" next to the embossment so you know it's there even if you cannot see it. If you see a stamp, a faint embossment, or an E, and the written expiry is in the future, check the box.

Any write-overs by the circulator or notary need to be lined out, rewritten, and initialed.

Signatures

Even though there is a birthdate on the petition, there are no birthdates in the voter file, so only address is searchable. Make sure the year is not off more than a decade or it is a non match.

County is required and must match what's written at the top of the form. Otherwise, tag it -o0.

City is required, not zip code.
